Nomination Instructions
You may submit nominations in as many categories as
you wish. Please obtain the consent of the nominee prior to submitting
the nomination. You may resubmit nominations from past years if your
nominee has not yet been honored with an award.
Nominations may be submitted
electronically (online or by email) or by completing a nomination packet. If you

Nomination Deadline

MUST BE SUMBITTED BY 11:59 P.M. ON MONDAY, SEPTEMBER 1, 2008 (ONLINE/EMAIL SUBMISSIONS) OR
POSTMARKED BY SATURDAY, AUGUST 30, 2008 (MAILED NOMINATIONS).

nomination process which includes:
- Nomination Application
- Narrative summary using the criteria questions
- Up to three optional supporting documents (letters of support, news clippings, etc.) may be uploaded. Acceptable formats include Microsoft Word (.doc), jpg, and tif. Individual files over 1MB and videos will not be accepted.
- Email Leilani Fortuno at lei@iolani.org to confirm the receipt of your nomination.
