Description of career achievements: length and tenure of fund raising background, experience, and results. (Recipient should have at least five years of professional fund raising experience).

Evidence of quality of leadership that is effective, creative, and stimulating, and evidence that the individual acts as a team player. (0-25 points)

Evidence that the nominee practices and promotes sound and ethical principles, following the AFP Code of Ethical Principles & Standards of Professional Practice. (0-15 points)

Description of individual's commitment to continuing professional development, conference participation, publication and professional education. (0-15 points)
