the nomination form (please nominate only one person per nomination application) and provide a summary narrative,
not to exceed 500 words, addressing the award criteria.
A maximum of 3 supporting documents (i.e. news clippings, letters of
recommendation, etc.) may be included, but should only enhance the
narrative and not be used in place of the narrative. The support
documents must be submitted together with the application. No videos
will be accepted.
